⚠️ Big Brand Name Confusions

“TriStar” sounds generic — many healthcare workers assume it’s a regional nonprofit. It is HCA Healthcare’s Tennessee brand. Every TriStar hospital is publicly traded for-profit.

“Encompass Health” sounds nonprofit — the word “Health” is a common nonprofit signal. But Encompass Health is NYSE: EHC, publicly traded for-profit. The largest IRF (inpatient rehab) operator in the U.S. — and almost universally NOT PSLF-eligible.

“Saint Thomas” is PSLF-eligible — unlike “Saint Mary’s” in Reno or “Saint Joseph’s” in some other cities (which may be Prime or Tenet), Saint Thomas in Nashville is genuinely Ascension Catholic nonprofit. PSLF-eligible.

The HCA TriStar Concentration Risk

If you’re a Nashville healthcare worker with student debt, your PSLF strategy must actively avoid the TriStar brand. Here’s the math:

  • Average dental hygienist at TriStar Centennial: $72k salary, $32k loan balance, no PSLF → pays full $32k + interest over 10 years.
  • Same hygienist at Vanderbilt Health: $69k salary (slightly lower), $32k loan balance, PSLF after 120 payments → potentially discharges $20–25k after 10 years on IDR.

Net PSLF benefit: $15,000–$25,000 over 10 years even with the $3k/year salary discount.

How to Verify Your Employer

  1. Check the W-2 EIN. Go to the IRS Exempt Organization Search (apps.irs.gov/app/eos). VUMC, Saint Thomas, Meharry — all appear as 501(c)(3) public charities.
  2. For TriStar / HCA jobs, the EIN search will not show 501(c)(3) status — confirming for-profit status.
  3. Submit the PSLF Employer Certification Form annually via StudentAid.gov/pslf. Don’t wait 10 years.

5 Action Items for Nashville Healthcare Workers

  1. VUMC and Saint Thomas are your two PSLF anchors. Most major Nashville healthcare jobs route through one or the other.
